Chuck Thompson Award

Nomination Form (2008)

All nominees must have a minimum of ten (10) years service with the University.  Nominees must have demonstrated extraordinary dedication/determination/results in the course of his or her daily work or in a “special project” situation. Two awards will be given:

One award will be given to an employee from the Crafts area.

One award will be given to an employee from the services, clerical and support staff area.

Nominations must be submitted no later than 5:00 pm, Friday, September 5, 2008.  Only one person should be nominated per form.
